How to score and review

Every risk is scored twice: once before controls (inherent) and once after (residual). The spreadsheet does the math and the color coding; your team supplies the judgment.

Rate likelihood and impact from 1 to 5
Use the shared definitions on the Scoring Guide sheet so ratings mean the same thing across teams. The inherent score (likelihood x impact) calculates automatically.

Record controls and name an owner
List the technical, contractual, and procedural controls that reduce each risk, and assign one accountable control owner per row.

Re-score for residual risk
Rate likelihood and impact again assuming controls operate as intended. A residual score still in the red band needs more treatment or a formal, senior-level acceptance.

Set status and review dates
Track each risk as Open, Mitigating, Accepted, or Closed. Review red risks monthly, amber quarterly, and green annually, and re-score whenever a tool, vendor, or regulation changes.
Low1 to 5

Acceptable with routine monitoring. Review at the standard cadence.

Medium6 to 14

Actively mitigate. Assign an owner, agree on a treatment plan, and review at least quarterly.

High15 to 25

Escalate to leadership. Treat before further rollout; consider pausing the activity until controls land.
